A birthmark is a mark that appears upon a person at birth. In most people, they appear as some form of defect upon the person's skin.

Empath: The Luckiest Smurf stories[]

In the EMPATH: The Luckiest Smurf story series, three known Smurfs of Empath's generation were born with birthmarks: Hefty, Empath, and Brainy. Hefty, being the first Smurf born of that generation, was given a heart-shaped mark that appeared on his right upper arm. Empath, born seven years later, was given a yellow star that appeared on his forehead. Brainy, born ten years after Hefty, was given a lightning bolt on his forehead that is hidden underneath his hat. While very little is known about what the heart mark symbolizes, the star mark symbolizes the emergence of telepathic and telekinetic abilities within the Smurf who bears that mark. Papa Smurf, who discovered Brainy's birthmark, was told by Paladore's spirit that it symbolized that Brainy had latent magical abilities that needed to be harnessed and disciplined so that he could be a powerful sorcerer.

Hefty would disguise the fact that he even had a birthmark at all by having Painter create an identical indelible mark on his left upper arm, referring to both marks as tattoos. Hefty's marks noticeably have changed to a yellow X when he had contracted the Yellow Hate Disease, only to revert to their normal appearance when he overcame the disease. In "Empath's Wedding," when Hefty gave up his heart for Smurfette so that she could feel love again, the heart marks of his shoulders (seen only on his tuxedo jacket) disappeared altogether, and did not reappear until Empath gave up his heart for Hefty. In an alternate timeline explored by Traveler and Empath in "Hefty, The Luckiest Smurf," Hefty's daughter through Smurfette would be born with a heart-shaped birthmark on her right upper arm.

Empath's birthmark usually glows whenever he uses his telepathic and telekinetic abilities, and can be made to disappear and reappear at will when Empath wishes to disguise himself as a normal Smurf.

On Smurfette Island, Empath's and Hefty's counterparts, Sympathy and Artemis, are also born with birthmarks.

Hero: The Guardian Smurf stories[]

Hero Stories

The first known character to have a birthmark in the HERO: The Guardian Smurf story series is Saviour Smurfette, the daughter of Hero and Wonder, who was born with a yellow H on her forehead, similar to that of both her father and mother. Her half-sister Miracle, born years later to Hero's second wife Smurfette, would also bear a similar birthmark. According to old legends, the H mark is actually known as the legendary "Mark of the Guardian".
