The Mirror Universe version of Hefty that exists in the EMPATH: The Luckiest Smurf story series considers himself the strongest Smurf of his generation alongside Tuffy and Duncan, just like his normal universe counterpart. However, the Mirror Universe Hefty is a brutal taskmaster with the Smurfs he works with and would only spare his fellow Smurfs if they need to be taught a lesson. He believes that he would be a better assistant to Papa Smurf than Empath is, but refuses to challenge Empath for the position knowing the kind of power he possesses. In addition to dating Smurfette, who in the Mirror Universe is married to Papa Smurf, Hefty also has a similar relationship with Handy, which nobody in the village has a problem with despite the fact that they are both biologically twin brothers.
He has a skull-and-crossbones tattoo on both his upper arms and wears an eyepatch due to the loss of one of his eyes.
